Hi!
I am no stranger to piercings, as I have many others, but I got my nape done near a month ago, and I am having trouble with it.
I am aware about the possibility of migration and other complications. I researched the piercing before I got it. I haven't had any trouble with any of my others, and I seem to heal pretty well, so I figured, why not?
I went to a parlor that specialized in surface piercings (a place I go to for all my other body mod needs, lol) and got it done by their senior guy. Now, we had a bit of trouble in the begining, since he couldn't get the clamp to stay on the back of my neck... I didn't have a lot of meat back there. In the end, he pinched it, and did it by hand with a straight barbell.
Personally, I thought it was a little shallow, but I had it looked at by another piercer at another parlor, and he said it was ok. There are apparently different styles that are used for that area, and I didn't have enough skin for a surface staple or curved barbell.
The first few weeks were fine. It seemed to be healing pretty well. I had my roommate clean it out for me every morning and every night with the same Dial anti-bacterial soap (thank god for roommates with strong stomachs, lol) that I used on my other piercings, and it was fine.
Within the last two weeks or so, it has gotten extremely red and tender to the touch, and has started to develope 'crusties' again around the rim of the actual piercing exits.
I really don't want to have to take this thing out, since I know that's not good if there's an infection in there, and I definately want to keep it. The piercer that did it for me is out of town currently, and unavailable, unfortunately. I haven't been able to contact anyone else, either, since most piercers in the area seem to be a bit territorial, and don't want to deal with anything they didn't personally do.
I am reluctant to put anything on it aside from cleaning it lightly with warm water when it seems to get really gunky.
Any recommendations?

Your piercing is migrating. You can either wait until it comes out on its own, or you can remove it now to minimize the scaring.
The problem we found is that the straight barbell does not allow for any flexibility. We use a staple barbell which is curved at each end and very small balls. We have also found flat head pieces that work even better as they reduce any friction that the balls would have.
We also found that cleaning this type of piercing is extremely delicate and only needs a very short cleaning period. Overcleaning this type of piercing increases the risk of migration.
Good luck
